Position Function:

The Registered Nurse Case Manager participates in treatment planning for clients with acute medical needs, schedules and coordinates medical appointments, reviews medical documentation, oversees medication ordering, refills, storage, and administration processes, communicates with the team about medical needs of each client, and conducts routine audits for all medication documentation. Ensures that all clients are being treated in accordance with the Psychotropic Medication Utilization Parameters for Children and Youth in Foster Care. The Registered Nurse Case Manager observes clients and assists in identifying and managing any side effects experienced due to medication.

The Registered Nurse Case Manager provides medical education for the agency to ensure medications and treatments for youth are conducted in accordance with approved techniques, licensing standards, and physician instructions. The Registered Nurse Case Manager ensures that all medical records, logs, and medical appointment documentation are accurate and maintained in the client file. The Registered Nurse Case Manager provides medically related functions necessary for the agency to fulfill the requirements of government regulations related to health and medication management.

Requirements:

Education Requirements: Associate or Bachelor of Science in Nursing and currently licensed by the State of Texas as a Registered Nurse.

Experience Requirements: At least one year of pediatric or psychiatric nursing experience.

Scheduling Requirements: Ability to work on-site in a residential setting Monday through Friday from 6:00 a.m. to 3:00 p.m.

Working Conditions:

Exposure to bloodborne pathogens, bodily fluids, illness, and injury. Frequent standing, walking, bending, and movement throughout the workday. Ability to respond appropriately in crisis situations and utilize approved crisis intervention techniques when necessary.

Maintain strict confidentiality and comply with all agency policies, HIPAA requirements, and applicable privacy regulations.

Key Expectations/Responsibilities:

Medical Case Management:

  • Assess resident medical needs and coordinate appropriate treatment, follow-up care, and physician communication.
  • Schedule medical appointments for clients and collaborate with the treatment team to ensure transportation plans are in place.
  • Attend psychiatric and specialty medical appointments with clients as needed.
  • Review intake files of all potential clients with documented acute and/or long-term medical needs.
  • Participate in treatment team meetings for clients with acute, chronic, or specialized medical needs requiring ongoing maintenance and monitoring.
  • Work collaboratively with program clinical teams, operational teams, and agency compliance teams to meet the individual medical needs of clients and uphold agency and DFPS policies, procedures, and guidelines.

Medication Management:

  • Oversee medication ordering, refills, storage, and administration processes to ensure clients receive medications as prescribed.
  • Administer medications as needed.
  • Monitor compliance with Psychotropic Medication Utilization Parameters for Children and Youth in Foster Care.
  • Observe clients for medication side effects and communicate concerns to appropriate medical providers and treatment team members.
  • Interpret physician instructions for direct care staff and ensure treatments are administered as ordered.

Training & Education:

  • Provide training to staff on meeting the medical needs of clients.
  • Provide education and support to clients regarding medical diagnoses, treatment plans, and medication management.
  • Support staff understanding of medication administration procedures, health-related protocols, and regulatory requirements.

Documentation & Compliance:

  • Conduct weekly audits of medication documentation, medical records, logs, and medication storage practices.
  • Ensure all medical records, appointment documentation, and health-related logs are complete, accurate, and maintained in client files.
  • Utilize agency documentation systems to maintain accurate and timely clinical records.
  • Support agency compliance with licensing standards, DFPS requirements, HIPAA regulations, and medical best practices.

Wellness & Support Services:

  • Consult with staff regarding nutrition, dietary concerns, and wellness-related issues.
  • Participate in 24-hour on-call rotations to address medical concerns.
  • Provide ongoing medical support and consultation to program staff regarding client health needs.
