This presentation will illustrate how effective aftercare supports and services for youth (and their families) transitioning from residential care, such as the Boys Town On the Way Home® (OTWH) aftercare model, can help youth maintain the gains they make in out-of-home care, keep them at home and in school, and prevent their return to residential care.
